Improved PvE lobby creation

Fixed an issue where PvE lobby creation could fail when the bot's guild member or role information was not available in the local cache. PvE creation now includes proper fallbacks to retrieve the required information directly from Discord.

PvE lobby creation now handles the required channel permissions more reliably. If lobby creation still fails, make sure the configured PvE category grants Blackwater View Channels, Manage Channels, and Manage Permissions.
